Page shell for anything printed on a thermal POS printer.
The paper width comes from the pos-printer-58mm global setting: 80mm rolls by default,
58mm when it is on. The page sets no height and no margin, so the printer driver's
roll-paper size decides where the paper is cut — pick the driver's receipt / roll size
(not A4) once in the print dialog and the browser remembers it.
Everything is printed solid black: thermal heads print grey as a dither that fades.
